A Team-Based Approach to Healthy Aging with HIV

Register NowAs the population of older adults living with HIV continues to grow, coordinated, multidisciplinary care is increasingly important to address the complex medical, behavioral, and social needs of this population. This webinar will explore strategies for integrating healthcare providers, case managers, behavioral health professionals, pharmacists, peer navigators, and community-based organizations to deliver comprehensive, person-centered care. Participants will examine common challenges to care coordination, opportunities for interdisciplinary coordination, and approaches to improving health outcomes, quality of life, and healthy aging among older adults living with HIV.
Learning Objectives:
- Identify the roles of multidisciplinary care team members in supporting the health and well-being of older adults living with HIV
- Explain strategies for improving communication, coordination, and collaboration across care settings and service providers
- Recognize opportunities to integrate medical, behavioral health, and supportive services to enhance patient-centered care and healthy aging outcomes.
Presenter:
Dorcas Baker is a registered nurse of 49 years, and previously employed by the Johns Hopkins University Regional Performance Site as the Regional Coordinator for the Mid-Atlantic AIDS Education and Training Center (MAETC). She holds professional membership with the Association of Nurses in AIDS Care (ANAC), and serves as a member of ANAC’s Expert Advisory Committee on HIV and Aging, and awarded the ANAC HIV Educator of the Year Award in 2019. She serves as a member on the scientific committee of the International Workshop on HIV and Aging. She is the founder of Older Women Embracing Life (OWEL), a support group for women over the age of 50, living with and affected by HIV/AIDS, the co-founder of the Maryland Coalition on HIV and Aging (MCOHA).
