Description
The SCORE (Sickle Cell Outreach Resources and Engagement) Program Community Health Worker (CHW) is responsible for helping clients and their families navigate and access community services and other resources to adopt healthy behaviors, maintain or retain care. The SCORE CHW supports providers and social service agencies with an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of clients with sickle cell disease (SCD) and/or their families. The CHW provides social support and informal counseling, advocates for individual and community health needs, and supports client goals.
Work Schedule and Salary
- 30 hours per week; $21.00 per hour.
- Hours and days worked are flexible and vary based on client and organization needs.
Location
CHWs will be hired to work in one of the following regions:
- Capital Region: Prince George’s and Montgomery County
- Central Maryland: Anne Arundel, Baltimore, Carroll, Harford, Howard Counties, and Baltimore City
- Eastern Shore Region: Caroline, Cecil, Dorchester, Kent, Queen Anne’s, Somerset, Talbot, Wicomico, and Worcester County
- Southern Maryland: Calvert, Charles, and St. Mary’s County
- Western Maryland: Allegany, Frederick, Garrett, and Washington County
Required Experience and Skills
- Written and oral fluency in English.
- Excellent communication skills, such as active listening and using language appropriately, verbally and in writing.
- Experience working as a CHW in a multicultural or community-based setting for a minimum of 2 years.
- Experience providing emotional support, encouragement, and motivation to clients.
- Experience working with many diverse people, including children and teenagers.
- Basic computer skills, word processing, and data entry.
- Effective telephone skills.
- Ability to work independently, be self-directed, and flexible.
- Ability to prioritize and perform functions with minimal supervision.
Preferred Skills and Abilities
- CHW Certification from an accredited training program.
- Electronic medical record (EMR) experience.
- Experience working with clients living with sickle cell disease.
- Good organizational skills to handle multiple priorities while remaining professional.
- Strong level of confidentiality due to the sensitivity of materials and information handled.
- Ability to make suggestions on workflow or system efficiency and effectiveness.
- Ability to work at a high-volume level of accuracy.
- Be skilled in computer/tablet use, especially in Gmail/Google Office Suite.
- Be skilled with web meeting platforms (e.g. Zoom).
- Experience in addressing harm reduction, health equity, social justice, and the social determinants of health, sexual and behavioral health, and related support services, including prevention, housing, care, and treatment.
Key Functions
- Responsible for establishing trusting relationships with clients and their families while providing general support and encouragement.
- Provide ongoing follow-up, basic motivational interviewing, and goal setting with clients/families.
- Follow up with clients via phone calls and visits to other settings where clients can be found.
- Help clients set personal health-related goals and attend appointments.
- Provide referrals for services to community agencies as appropriate.
- Help clients connect with transportation resources and provide appointment reminders.
- Exhibit excellent working relations with clients, visitors, and staff.
- Work closely with medical providers to help ensure that clients have comprehensive and coordinated care plans.
- Work collaboratively with other clinical personnel assigned to the same client.
- Knowledgeable about community resources appropriate to the needs of clients/families.
- Responsible for providing consistent communication to involved parties, ensuring that provided information and reports clearly describe progress.
- Act as a client advocate and liaison between the client/family and community service agencies.
- Record client care management information in software no later than 24 hours after client contact.
- Attend regular staff meetings, training, and other meetings, as requested.
- Manage an assigned caseload of clients.
- Maintain HIPAA compliance at all times.
- Performs other duties as assigned.
- Travel as required within one of the five regions of Maryland, as defined above.
Pre-Employment Requirements
- Resident of the state of Maryland.
- Completion of pre-employment drug testing and background check.
- Quiet, internet-enabled, private, remote workspace.
- Valid Maryland Driver’s License with a clean driving record.
- Vehicle for personal use or reliable transportation. Reliance on public transportation is not permitted.
Education Requirements
- High school graduate or equivalent.
- Successful completion of a Community Health Worker training program, such as from a college or other educational institution, is preferred.
- Medical terminology and/or background preferred.
While performing the duties of this job, the CHW is regularly required to sit for prolonged periods, talk, and listen. The CHW is occasionally required to stand, walk, and use hands to operate a computer keyboard. The employee may occasionally lift and/or move up to 25 pounds. Vision requirements include: close vision, distance vision, depth perception, and the ability to adjust focus. Ability to read multiple handwritings.
